Description of Columns:
- Incident ID: A unique identifier for each incident.
- Date: The date on which the incident occurred.
- Location: The specific site or area where the incident occurred.
- Type of Incident: A brief description of the incident (e.g., fall, electrical shock).
- Type of Injury: The nature of the injury sustained (if any).
- Days Off: The number of workdays the injured employee was absent.
- Immediate Corrective Actions: Actions taken immediately following the incident to prevent immediate reoccurrence.
- Long-Term Corrective Actions: Strategic measures implemented to prevent future incidents of a similar nature.
- 3-Month Follow-Up: Actions and observations noted three months after the incident.
- 6-Month Follow-Up: Actions and observations noted six months after the incident.
- 9-Month Follow-Up: Actions and observations noted nine months after the incident.
- Status: Current status of the incident report (e.g., Open, Monitoring, Closed).
Instructions for Use:
- Filling the Log: Enter all relevant details immediately after an incident occurs.
- Updating: Regularly update the log with information from follow-up actions and observations.
- Reviewing: Periodically review the log to identify any trends or recurring issues that may require systemic changes.
